Alpha-GPC is a compound that delivers choline, a building block the brain uses to make acetylcholine, a key memory chemical. It is used as a supplement and, in some countries, as a prescription for cognitive decline. The best human trial added it to a standard Alzheimer's drug and suggested a benefit for symptoms; overall evidence is moderate. It is generally well tolerated. In the U.S. it is a dietary-supplement ingredient, not an approved drug.
The science
Alpha-GPC (choline alfoscerate) is a cholinergic precursor that raises brain choline availability for acetylcholine and phospholipid synthesis. In the randomized, double-blind, placebo-controlled ASCOMALVA trial, adding alpha-GPC to the acetylcholinesterase inhibitor donepezil in Alzheimer's disease produced greater cognitive and functional benefit than donepezil alone over the study period, including effects on apathy and, on imaging, regional brain volumes (Amenta et al., 2012; later ASCOMALVA reports). More recent work continues to explore choline alphoscerate in mild cognitive dysfunction (Carotenuto et al., 2024). It is prescription-available in some countries for cognitive disorders but is a supplement in the U.S. and not FDA-approved as a drug; note observational signals linking high supplemental intake to cardiovascular risk warrant discussion.
